KALAMAZOO, MI (WKZO AM/FM) – Ascension says the health system has been hit with a data breach in December that included demographic information including names, addresses, phone numbers, email addresses, dates of birth, race, gender, and Social Security numbers of patients.
The information accessed also included clinical information related to an inpatient visit, such as the place of service, doctor name, admission and discharge dates, diagnosis and billing codes, medical record numbers, and insurance company names.
Ascension says they are offering two years of complimentary credit monitoring and identity theft protection services to people impacted.
Their investigation determined Ascension “inadvertently disclosed information to a former business partner, and some of this information was likely stolen from them due to a vulnerability in third-party software used by the former business partner.”
The health system says the incident did not involve their systems, networks, or electronic health records.
